Our role (and yours) in Patient Centered Care

More than a buzzword, “patient-centered care” has notably shifted the healthcare approach from a physician-dominated dialogue into a more collaborative mindset where patients are active participants. It’s a philosophy that pulls away from medicine motivated solely by productivity to that based on care through the patient’s eyes. The Institute of Medicine formally defines this as: “Providing care that is respectful of, and responsive to, individual patient preferences, needs and values, and ensuring that patient values guide all clinical decisions.”

This is the approach we take at Manchester Care Homes. And we all have a role to play – even family and friends. How patient-centered care looks in practice is outlined in its eight core principles.

1. Respect for patients’ values, preferences, and expressed needs. Each of our residents as individuals and are involved in the decision-making process, bringing his or her own values, preferences, and cultures into their care plan.


2. Coordination and integration of care. Our residents are served by dedicated caretakers who assist with the coordination of clinical care, front-line patient care, and all support services.


3. Information and education. Our residents are informed about their prognosis and progress along with care processes, so they rest easy knowing what to expect, when.


4. Physical comfort. We keep our residents comfortable with the care that incorporates assistance with daily living, pain management, and comfy indoor and outdoor spaces.


5. Emotional support and alleviation of fear and anxiety. Our residents receive the emotional support they need when it’s needed. This includes compassionate support regarding financial issues, their own wellness, and the impact of illness on their families.


6. Involvement of family and friends. Loved ones play an integral role in patient-centered care, which is why we include family and friends in the decision-making process. We also provide friends and family with support, information, and accommodations.


7. Continuity and transition. Residents receive much-needed assistance, guidance, and coordination when the time comes for discharge, care financing, and supplementary care to ensure smooth and uninterrupted treatment.


8. Access to care. Our residents are given the information and help they need to access care – from assistance in exploring and navigating all options available to scheduling appointments and planning transportation to and from.
